YAI NETWORK MEMBERSHIP APPLICATION FORM

Dear YAI Network Members and Partners,

Warm greetings from the Young & Alive Initiative (YAI) Network Steering Committee.

Since the establishment of the YAI Network, we have shared the membership form twice, most recently in 2024, to onboard and document individuals and organizations engaged in advancing youth-led work on sexual and reproductive health and rights (SRHR) and related thematic areas.

As the Network continues to grow and evolve, we are currently updating our membership database to ensure we have accurate, up-to-date information for all members. In this regard, we kindly invite both existing and new members to complete the YAI Network Membership Form 2026.

Completing this form will help us:

• Maintain an updated and inclusive list of YAI Network members

• Strengthen coordination, communication, and collaboration within the Network

• Identify opportunities for learning, advocacy, partnerships, and collective action

Please note that all current members are encouraged to fill out the form, even if you have completed a similar form in the past, as this will replace previous records.

If you are new to the Network, this form also serves as an official expression of interest to join the YAI Network.

Ndugu Wanachama na Washirika wa Mtandao wa YAI,

Salamu za dhati kutoka Young & Alive Initiative (YAI).

Tangu kuanzishwa kwa Mtandao wa YAI, tumeshawahi kusambaza fomu ya uanachama mara mbili, mara ya mwisho ikiwa ni mwaka 2022, kwa lengo la kuwatambua na kuwaunganisha watu binafsi na taasisi zinazofanya kazi katika masuala yanayoongozwa na vijana, hususan Afya na Haki za Uzazi na Ngono (SRHR) na mada zinazohusiana.

Kadri Mtandao unavyoendelea kukua na kupanuka, tupo katika mchakato wa kusasisha kanzidata (database) ya wanachama wa Mtandao wa YAI ili kuhakikisha tunakuwa na taarifa sahihi, za sasa, na zinazowakilisha vyema wanachama wetu wote.

Kwa muktadha huo, tunawaomba kwa heshima wanachama waliopo na wanachama wapya kujaza Fomu ya Uanachama wa Mtandao wa YAI kupitia kiungo kilicho hapa chini:

Kujaza fomu hii kutatusaidia:

• Kuwa na orodha iliyosasishwa ya wanachama wa Mtandao wa YAI

• Kuimarisha mawasiliano, uratibu, na ushirikiano ndani ya Mtandao

• Kutambua fursa za pamoja za kujifunza, kufanya utetezi, ubia, na hatua za pamoja

Tafadhali zingatia kuwa wanachama wote wa sasa wanahimizwa kujaza fomu hii, hata kama uliwahi kujaza fomu ya uanachama hapo awali, kwani taarifa hizi zitachukua nafasi ya rekodi za zamani.

Kwa wale wapya, fomu hii pia itatumika kama ombi rasmi la kujiunga na Mtandao wa YAI.

Iwapo utakuwa na maswali au utahitaji ufafanuzi zaidi, tafadhali usisite kuwasiliana nasi.

Tunathamini sana mchango wako na tunatarajia kuendelea kushirikiana nawe katika kuwawezesha vijana na kuendeleza masuala ya SRHR nchini Tanzania.
